Frontend user not working on login

Description
The problem is for versions 2.1 AND 2.2 and includes:
1. inability to view users in admin 2. when user is created, it is added to database but you get non comfirmation in admin 3. If you add a new user to a group with no permission, and then login using that user account, the URL redirects to /admin, then back to login and you are not logged in 4. It appears that the members, group and member_groups tables work fine and have the correct relationships and the right id's etc are created and connected.
