Allow exporting of data from a UserDefinedForm

Description
Add a "Export form submissions to excel" link on the UserDefinedForm? Submission tab.
I believe this has been done for some of our clients already (e.g. meridian?).
The usefullness of a user defined form increases dramatically with this feature. I'm sure this used to be a feature request in trac a year ago, but cannot find it.
